40 original interactive cases: 20 with a 10-minute real-time limit and 20 with a 20-minute real-time limit. Search and place labs, imaging, medications, fluids, blood products, procedures, consultations, monitoring, counseling, and follow-up orders. Simulated clinical time advances separately from the real-time clock. The 20/20 practice split is an internal educational design choice; USMLE does not publish a required short/long case ratio.
